Financial & Insurance Information

At Melville Dentistry we believe you deserve quality dental care and patient service, provided by a professional staff with the highest level of expertise. We strive to make our services affordable to a large number of patients, some with dental benefits and some without. For patients who do not have a dental insurance plan, we offer our own, in-house plan. For information on our plan, visit our Specials web page. The information listed below outlines our dental practice policies relating to insurance coverage and payment for services:

  • In order to provide affordable access to our services, we are participating providers with Guardian, Delta Dental, Cigna, and United Concordia.
  • For all other insurance carriers, such as Met LIfe, AETNA, Blue Cross Blue Shield and others, we will accept the assignment towards your treatment, and we will work with you to maximize your dental benefits.
  • Your dental benefits are based upon a contract between your employer and an insurance company. If you have any questions about your plan and coverage provided, please call your insurance company or contact your employee. Dental benefit plans do not pay for complete dental care — patients are required to pay for a portion of their dental care.
  • Each dental plan has a unique schedule of coverage. If you would like to know how much the insurance benefits will be, we can file a “pre-treatment authorization” with your insurance company. Keep in mind that this is not a guarantee of payment and it will delay treatment considerably.
  • We will bill your insurance as a courtesy.If the insurance does not pay within 90 days, we reserve the right to request payment in full for services and let you collect the insurance funds due to you. This is rare, but be aware that the insurance benefit you have is a legal contract between you and the insurance company. Our office is not, and cannot be, part of that contract.
  • Melville Dentistry does require payment of your portion at the time of service. We accept Master Card, Visa, cash and checks. Our office offers a 3, 6 and 12 month interest free financing(same as cash) through Care Credit, a third party financing company. Longer terms are also available, at a slight interest rate. Please consult with our financial coordinator for details.
  • A specific amount of time is reserved for you. We strongly urge all our patients to keep their appointments. If you need to change your appointment, we require at least a 24h notice to avoid a $35 /hour cancellation fee (emergencies are an exception).
  • In the event of an emergency after regular business hours, a $75 emergency fee will be charged for established patients in addition to the treatment fee. Patients not established in the practice will be charged $125 after hours emergency fee.
